The requirements below must be completed annually:​

• Students must create an aktivate account and complete the online sports registration prior to trying out.

• Students must pass a physical examination. The physical examination must be completed on the approved NIAA Physical Form.

• Students must return the Medical Eligibiliy Form signed by the physician (yellow sheet in the packet). 

• Students must return the Release Form signed and dated by parent and student (green sheet in the packet).

• Students must return the Concussion Acknowledgement Form signed and dated by parent and student (pink sheet in the packet).

• Students must maintain an unweighted grade point average of at least 2.0.

• Students must maintain a passing grade in all classes during the season in which they are participating.

• Students must not be involved in any major behavioral incidents.

• Students are required to have accident and health insurance on their own prior to participation in any sport or school sponsored activity.​

• Students enrolled through Change of School Assignment (COSA) or Shared Housing are ineligible for 180 school days.

A student will not be allowed to try out for any of our sports or non-sanctioned activities without the above requirements completed. Please see Mrs. Romero if you have any questions.
